Team TJ Donations

Thank you for donating to help our son, TJ, and to support our efforts to cure children’s brain tumors. Along with other parents, we have established our own foundation, the Pediatric Low Grade Astrocytoma Foundation (“PLGA Foundation”), that will host the 2010 Team TJ event - The 2010 Geared Up For Kids Bike Ride. Our 501(c)(3) charitable organization directly funds children’s brain tumor research and details on our foundation and our medical research can be found at www.fightplga.org.

If you prefer to donate by check, please make the check payable to: Team TJ / PLGA Foundation.

Also, please complete this form with your check so we can be sure to credit your donation correctly.

Donating to Team TJ through the Fidelity Charitable Gift Fund

Donating Online with Fidelity Charitable Gift is easy:

Go to charitablegift.org. Then “log-in.”
On the Giving Account Summary page, use the pull down feature on the “Take an Action” menu and choose “Grant Recommendation.”
Type in the following Tax ID #: 260295572 and hit “search.” The “Pediatric Low Grade Astrocytoma Foundation, Chappaqua, New York” will pop up below as a search result.
Click on “select” next to the “Pediatric Low Grade Astrocytoma Foundation” in the “Recommend a Grant” column.
Use “Amy Weinstein” as the contact name, with the following phone number: 914.762.3494
In the “Designate Special Purpose” field please enter the following in the text box: “Team TJ / Geared Up For Kids 209 Ride”
On “Specify Timing” page, click “as soon as possible”
The rest is the same as any other grant request.

Provide the following Tax ID#: 260295572. You can also tell them you are providing a grant to the Pediatric Low Grade Astrocytoma Foundation located in Chappaqua, New York.
Please be sure to tell the representative you want this to be designated for a “Special Purpose.” The special purpose is “Team TJ / Geared Up For Kids 2009 Ride Donation.” Ask them to use the following name as the contact person “Amy Weinstein.” If they ask for her phone number it is 914.762.3494.
The rest is the standard process.
